Abstract

The rapid integration of digital twin technologies within healthcare ecosystems has transformed the operational architecture of cyber-physical healthcare infrastructures. Digital twins enable real-time synchronization between physical healthcare entities and their virtual counterparts, thereby supporting predictive diagnostics, patient monitoring, intelligent decision-making, and resource optimization. However, the increasing dependence on interconnected Internet of Things (IoT) devices, cloud-edge infrastructures, and distributed healthcare systems introduces substantial cybersecurity risks, data integrity concerns, and privacy vulnerabilities. Existing healthcare digital twin frameworks frequently lack integrated security architectures capable of simultaneously addressing predictive analytics, blockchain-enabled trust management, and cyber resilience. This research proposes an intelligent unified model for digital twin security that combines blockchain technologies, predictive analytics, and adaptive cybersecurity mechanisms to enhance healthcare cyber resilience. The study synthesizes concepts from IoT healthcare systems, blockchain consensus structures, distributed ledger technologies, edge-cloud computing, and anomaly detection frameworks to establish a secure and scalable architecture for intelligent healthcare monitoring. The proposed model incorporates blockchain-based authentication, AI-driven predictive analysis, secure task offloading, anomaly detection, and decentralized healthcare data governance. Furthermore, the study critically evaluates security threats, operational
constraints, and implementation limitations associated with healthcare digital twins. The findings indicate that integrating blockchain-enabled verification mechanisms with predictive healthcare analytics significantly improves data integrity, cyber resilience, and operational transparency while reducing risks associated with centralized architectures. The research contributes a publication-quality framework suitable for next-generation healthcare cyber-physical infrastructures and provides strategic directions for future digital twin security research.
